Each page is checked for text and for anything printed on it. A page with no text and nothing visible on it is marked as empty. Very small marks are ignored, so a scanned page with only dust or a few specks still counts as empty.

Yes. Scanned blank pages are rarely perfectly white. Faint text showing through from the other side of the paper, light scanner noise, shadows along the edges, and punch holes are ignored, so these pages are found too.
